Rheumatoid arthritis (RA) is a chronic inflammatory disorder that primarily affects joints, leading to pain, stiffness, and swelling. Early and accurate diagnosis is crucial for effective management. Recent updates from the American College of Rheumatology (ACR) and European League Against Rheumatism (EULAR) have refined the criteria for diagnosing and managing this condition.
Patients suspecting RA typically exhibit constitutional symptoms such as morning stiffness, elevated acute phase reactants, and small-joint arthritis affecting the hands and feet. Notably, joint erosions appear late in the disease process. The diagnosis is primarily based on clinical symptoms, including joint pain, stiffness, and swelling lasting more than six weeks, swelling in three or more joints, morning stiffness exceeding 30 minutes, symmetrical symptoms, and swelling in the wrists, hands, and finger joints. Rheumatoid nodules under the skin may also be indicative of RA.
The 2010 ACR/EULAR Classification Criteria for RA introduced serologic markers, significantly enhancing diagnostic accuracy. These criteria encompass four domains, offering a comprehensive approach to RA diagnosis. The guidelines emphasize the importance of early treatment to slow disease progression and improve quality of life.
The ACR also provides diagnostic guidelines for osteoarthritis, including clinical classification criteria for the hip, knee, and hand, as well as guidelines for medical management.
In 2022, the ACR and EULAR revised their remission criteria for RA, with the aim of providing a more robust framework for assessing disease activity and treatment response. These updates are critical for clinicians in tailoring treatment plans to individual patient needs.
